He is survived by two sons, Patrick Leavens and Timothy Leavens; two grandchildren, Alyssa Leavens and Abigail Porch; one great-grandkids, Grayson Leavens.
The family will receive friends at Clark Funeral Home on Friday, October 21, 2022, from 12:00 PM to 2:00 PM. Graveside Service for Dwight will be held at Peaceful Meadows Memorial Cemetery on Friday October 21, 2022, AT 2:00 PM.
In lieu of flowers, the family requests donations be made in Dwight's honor to help with Funeral costs.
